The biggest barrier to conversion is, probably, overthinking it. Grab the monster, convert the HD and AC, look and see if you think that's reasonable, and adjust. Then take its regular attacks (rinse, repeat). With monsters from the 2e/3e era, I try and take the "defining" special attacks/...

They way I handle this is similar to Karaptis, but I let the _characters_ decide. I have hooks/rumors related to each adventure laid out in a table beforehand and the characters decide from the information they manage to take in. I'd second Prince Charming, though. My experience with the Hex Crawl stuff is limited to the first one -- The stats/gamey stuff is well separated from the rest of the text, so it's super easy to find and convert. I'd second Swords and Wizardry. It was perfect for my group since we play both S&W and DCC.

I do a grimoire at my table, where the player _writes down_ the result of the spell when they actually achieve that result. It's a little weird, but I like it that way.

something I haven't seen in RPGs (Savage Worlds has a bit of this) is the "kick factor" for firearms. There should be a STR minimum for using certain weapons (even modern ones). Early firearms were not easy to handle, especially for the small/weak.
